Temple Dungeon (extended version)

Modifications have been made to this version so
it sounds right in Unreal Tournament's terrible
sound engine:

Section 1 and 2:
Replaced M00 and N10 with vxx
Replaced Qxy with Q0y and vxx --
    except where this wouldn't work.

Section 5:
Replaced note cuts by removing unnecessary
    parts of the vxx crescendos.
