Charles, I am dissapointed that you have not tried the first very basic step: Google!
Click here (http://www.google.co.uk/search?hl=en&q=how+to+convert+MIDI+to+mod&btnG=Google+Search&meta=)
You can also use Modplug Tracker to export/import MIDI data.

the version there at sonicspot is aaaaages old. the latest version of the original Modplug Tracker is 1.16 and can be downloaded at modplug.com. however, since Olivier Lapicque released the sourcecode in 2003 or 2004, there has been created a new branch: OpenMPT (also downloadable at modplug.com). Since then, some modifications to midi import/export have been made, i believe.
