Upload declaration?

Hi, I've made a remix module of a tune originally made by Weird Bananas that I'm going to upload to TMA. I've also got the permission from him to do that, but what declaration option should I choose then?

As nikku says, declare it as your own - then it will end up on your profile as well. I have to admit that the current wording may be a bit too "harsh" to make this obvious.
