BRT (BeRoTracker)
SUNVOX (SunVox :P)
BBSONG (Beepola) (I don't know if it's module format)
SC (Sound Club)

As a rule of thumb, any newly added module format requires to be documented and have an open-source implementation. Otherwise we cannot scan uploaded modules, we cannot play them online, and we want all modules to be easily playable on pretty much any platform. That rules out several formats you mentioned.
